A valid prescription needs to include the following information:
- Prescriber's information (Name, Address, Phone#)
- Valid expiration date (within 2 years or less from date of exam)
- Pupillary Distance (PD) measurement
Oftentimes, your prescription will not include PD measurement, which is needed for manufacturing your glasses. Please refer to the link below for instructions on taking PD measurement yourself.
